While exploring the city's old storm sewers you get caught as a sudden storm starts filling the system. Your only hope is to find a manhole. As you frantically look you spy something in the distance. Can it be? Yes! It is a ladder! Now you have to reach it and open the manhole before the waters rise to the roof of the system. Can you do it?

One of the expanded versions of the old Collossal Cave text adventure involved exploring a set of sewers, and while in there your character was at risk of drowning if you were in the wrong place at the wrong time.

Your image reminds me of just that!
